Please select this if you need repeat sick note for work - this must have been approved by the doctor in a consultation previously. This fee does not apply to social welfare certificates.
Please select the type of blood test you are paying for - standard or private consultant blood test. If booking private consultant blood test there is an additional administration fee. In certain circumstances, higher fees apply, if you are not sure, please contact the practice to clarify.
Please select this option to pay for a routine regular prescription for medication you are already taking- please fill out the medication you are on in the message section and the pharmacy the prescription is to be sent to. Please allow 48 hours for this prescription to be processed during busy times.
